RE: Notice of Cancellation and Demand for Refund, Peak Fitness monthly membership

Dear Peak Fitness Clubs LLC,

I subscribed to Peak Fitness monthly membership (account M-55812) on January 15, 2025. On July 1, 2026 I requested cancellation by website. After that date I was charged $49.99 on August 1, 2026, $49.99 on September 1, 2026.

Idaho Code § 48-603G requires a seller of a subscription that renews automatically and was entered into online to provide a cost-effective, timely and easy-to-use online way to cancel, without an extra fee to cancel; if a phone number is offered for cancellation, it must be toll-free.

For online subscriptions that renew for terms of 12 months or more, the seller must send a renewal notice 30 to 60 days in advance that describes the goods or services, states the price, and gives at least one free way to cancel.

Please treat this letter as written confirmation that my subscription is cancelled, and stop all further charges. I also request a refund of $99.98 for the charges made after my cancellation, by October 16, 2026, by check mailed to the address above.

I would prefer to resolve this without further steps.
